Therapeutic adherence in hypertensive patients from an Ecuadorian rural community: a cross-sectional study

Arterial hypertension constitutes a global public health problem, with lack of adherence to pharmacological treatment being one of the main causes of poor disease control. This study aimed to identify the level of pharmacological adherence in hypertensive patients from the Barrio Lindo sector (Paquisha parish, Mocache canton, Ecuador). A cross-sectional study with a quantitative approach was carried out on a sample of 60 hypertensive patients selected through probabilistic sampling from a population of 640 inhabitants. A structured survey based on a five-level Likert scale, validated through specialized literature review, was applied. Results showed that 83% of patients take their medication at the established time, 92% respect the indicated dose, and 78% comply with treatment without family supervision, indicating a high adherence level. However, 51% reported having forgotten to take their medication at some point, 30% have voluntarily abandoned treatment, and 27% face economic difficulties to acquire the drugs. It is concluded that, although there is a favorable general level of adherence in the studied population, significant barriers related to forgetfulness, voluntary abandonment, and economic limitations were identified, which require educational and social support interventions to improve disease control.
